Project Manager – Primary Care FHT TC LHIN

Key Responsibilities

• Support Primary and Community Care Committee (PCCC), subcommittees and working groups
• Support Primary Care Clinical Lead (PCCL) on primary care initiatives
• Monitor performance and report on activity
• Support primary care providers on adoption of TC LHIN's approved initiatives
• Provide project management expertise
• Organize and conduct stakeholder engagements
• Coordinate development and monitoring of priority areas and related projects
• Prepare business cases, position papers, briefing notes, and other documentation
• Support other TC LHIN and local sub-region initiatives

Requirements

Education

Master's Degree in health or business administration

Experience

5 plus years related management experience

Required Skills

• Understanding of Ontario health care system and primary care delivery models
• Relationship building with local health service providers
• Project management expertise
• Excellent written and verbal communication skills
• Strong planning and organizational skills
• Analytical, problem solving and decision making skills
• Data analytics and epidemiology knowledge
• Ability to travel within sub-region
