Key Responsibilities
- Conduct comprehensive psychosocial and family assessments
- Support patient and their family by providing education, short-term counseling and crisis intervention
- Provide proactive, holistic support for Indigenous patients, parents, family/supporters
- Facilitate complex discharge planning
- Establish and maintain effective collaborative relationships
- Participate in program planning and development
- Promote Indigenous centered and family centered care
- Act in public relations and advocacy capacity
Requirements
Education
Master's Degree in Social Work
Experience
Minimum 1 year recent related experience
