  • Provide proactive, holistic support for Indigenous patients, parents, family/supporters
  • Facilitate complex discharge planning
  • Establish and maintain effective collaborative relationships
  • Participate in program planning and development
  • Promote Indigenous centered and family centered care
  • Act in public relations and advocacy capacity

Requirements

Education

Master's Degree in Social Work

Experience

Minimum 1 year recent related experience
