Social Worker, Integrated Care Team/DTES

Key Responsibilities

• Address client and family centred care needs
• Coordinate plan of care across multiple sites, clinics, and services
• Care for clients experiencing health challenges
• Identify problems and address issues in collaboration with interdisciplinary team
• Act as a resource for the DTES integrated team
• Provide direct care activities
• Establish and maintain therapeutic relationships

Requirements

Education

Master's Degree in Social Work

Experience

Two (2) years' recent related experience

• Conflict resolution
• Computer literacy

Required Certifications

• Current full registration with the British Columbia College of Social Workers
• Valid BC Driver's License (preferred)
