Key Responsibilities

Lead behavioral health assessments and develop therapeutic intervention strategies for PACE program participants
Serve as behavioral health advocate on interdisciplinary clinical team
Design and implement individual and group therapy programs including art, music, and psychosocial interventions
Manage comprehensive care planning and documentation for assigned participant caseload
Provide staff training and education on behavioral health interventions
