Key Responsibilities

Lead complex behavioral health case management for crisis intervention with youth up to age 20
Conduct comprehensive clinical assessments and develop strategic care plans
Coordinate with healthcare providers and community resources to optimize treatment outcomes
Manage crisis intervention and follow-up support services
Drive care coordination across multiple stakeholders including families and formal/informal support networks

Requirements

Education

Master's Degree in counseling, social work, or other related field

Experience

3+ years direct clinical practice post-Masters, 3+ years working with children/adolescents, 2+ years computer systems experience
