Key Responsibilities

Lead advanced illness management and palliative care delivery through comprehensive patient assessment and treatment planning
Develop and execute individualized care plans in collaboration with physicians and specialists
Manage complex medical cases requiring advanced clinical judgment and decision-making
Coordinate interdisciplinary care teams to ensure optimal patient outcomes
Prescribe and manage medications including controlled substances
Conduct regular care plan evaluations and modifications based on patient response

Requirements

Education

Masters of Science in Nursing from an accredited school of nursing

Experience

Minimum of two years Nurse Practitioner experience
