Key Responsibilities

Conduct comprehensive psychosocial and risk assessments in acute and subacute settings
Provide clinical supervision to Grade 1 Social Workers and students
Develop and implement patient-centered discharge planning
Collaborate with multidisciplinary healthcare teams
Deliver emotional support and interventions for patients and carers
Manage complex cases across multiple medical conditions
Coordinate with internal and external stakeholders for patient care

Requirements

Education

Bachelor Degree or Qualifying Masters in Social Work

Experience

Grade 1 Social Worker with hospital experience or current Grade 2 Social Worker
