Key Responsibilities

Lead clinical assessments and treatment planning for complex mental health and substance use cases
Coordinate interdisciplinary care delivery across multiple program areas including Complex Care Housing and Acute Care settings
Provide crisis intervention and emergency mental health services in various clinical settings
Develop and implement rehabilitation programs for diverse patient populations

Requirements

Education

Bachelor of Social Work degree (B.S.W.) from an accredited school of Social Work, OR a Master's degree (M.S.W.)

Experience

Related social work experience, preferably in a medical setting
