Key Responsibilities

Lead case management for Veterans Court program including eligibility assessment and program monitoring
Develop and implement individualized support plans for veteran rehabilitation
Coordinate with multi-agency stakeholder network including courts, VA services, and healthcare providers
Conduct crisis intervention and provide trauma-informed support services
Prepare detailed court documentation and progress reports
Advocate for veterans' needs across social service systems
Perform home visits and community outreach as needed

Requirements

Education

Bachelor's degree in Social or Behavioral Sciences

Experience

Two (2) years experience in social services related work
